REGISTRAR
SENIOR ASSOCIATE DEAN, EDUCATIONAL AFFAIRS, COLLEGE OF MEDICINE

RE: Redelegation of Authority - Duplicate Diplomas and Fees

The authority to issue and establish fees for duplicate diplomas was delegated to Chancellors by Presidential Delegation DA 0734, June 24, 1982 [superseded by DA 2559, April 12, 2011]. I am redelegating to the Registrar and the Senior Associate Dean, Educational Affairs, College of Medicine, the authority to issue a duplicate diploma in case of the loss or destruction of the original; or, in the case of a request on the part of a graduate whose name has been legally changed and, for good cause, wishes a duplicate diploma giving effect to the change of name.

Authority is delegated to the Registrar and the Senior Associate Dean, Educational Affairs, College of Medicine to establish fees for issuance of duplicate diplomas with the understanding that the Office of the President shall be informed of such fees for publication in the Student Fees and Deposit Circular.

This authority may not be redelegated further and supersedes the August 6, 1985 delegation of authority from Chancellor Peltason on the same subject.
